How do you ask a high school football player to sadies?

Well I've been talking to this guy for 3 months at my high school . Me and him have a relationship really strong . && our school dance called sadies is coming up . That's when the girl asks the guy. && i want to ask him , but I'm afraid to because I don't know how to ask him . Please help me !!
